Submit a New Incident Report
You can report an incident easily and quickly using the qHlas online form.
By submitting the form, the reporting person acknowledges that they are knowingly making a truthful report.

Complete the qHlas form.

Fill in all required information (marked with an *). Describe the incident clearly and factually, without using vulgar language. If the reporting person does not provide their first name, surname and date of birth, or other information from which their identity can be determined, they will not be entitled to protection under the Whistleblower Protection Act. If you have any relevant evidence relating to the incident, you can attach it in the Incident Attachments field by clicking Select files. If you provide your email address, you will receive a notification directly by email whenever the status of your incident changes.

Submit the form.

After submitting the qHlas form, you will be shown a link that you can use to track the status of your reported incident. Save the link and do not share it publicly. If you provide your email address, the link will also be sent to the email address you entered.

To check the status of your reported incident, use the link displayed after you submitted the incident. If you provided your email address when reporting the incident, you will receive a notification whenever the status changes. The status change will be displayed in the upper-right corner.

Under the Whistleblower Protection Act, the organization has 30 days to resolve the incident. In serious cases, this period may be extended. You can access the outcome of your reported incident again using the link displayed after submission or sent to the email address you provided. The outcome of the resolved incident will be displayed below the form.
“In accordance with Section 9(2)(a) of the Whistleblower Protection Act, TIPA, spol. s r.o. excludes the acceptance of reports from persons who do not perform work or other similar activities for the company as defined in Section 2(3)(a), (b), (h) or (i) of the Whistleblower Protection Act (i.e. dependent work performed under a basic employment relationship, service, voluntary work, professional practice or internship).”
